That Is not the right. Document of identification is your passport not your assento de Nascimento. If you put wrong info they won't give you an appointment.

Shayne when I filled the form in the Collum documents of identification I put that assent De Nascimento number and In description I put Indian passport number so is there will be any problem?

Yes because without the passport they wont let you in so when they book the appointment the passport number is stored in the system, so that only document used to identify you.
