Ah yes! One more thing, Adding of middle name is considered as major change in name. You can check out the information on the passport website and what documentation is required is all there. For eg. If your name is Sandeep and you want to change it to Sandip then that's a minor change no documentation is required for that. Alternatively drop in an email or call up the passport helpline number they will tell you if you can get the needful done only on the basis of your Birth certificate and Marriage Certificate. Hope both those documents that you have with you are Government registered documents and not church documents as those are not accepted at all.

If you have any government documents like birth certificate and marriage certificate plus any documentation like pan card with your mother's correct name (you can get your mother's name printed on your pan card now)they may accept. But they will ask you for her passport to check her name there. If on your mother's passport her middle name is not mentioned they will not accept. I've just got all these changes done four months ago so I'm well versed with all documents the passport office asked me. I asked the passport office to put her documents on hold twice because I wanted to be doubly sure. Try your luck though. It depends on who you get at the passport office. It's like a gamble.

Yes, that’s exactly what I was writing. I am not changing my mothers name only asking to add her existing middle from her BC into my passport. Yes marriage certificate is there.

Hi, since you are not changing your mothers name, you are not required to produce any other document except your birth certificate to the passport officer. Mam, try it.Also keep parent's marriage certificate handy if passport officer may demand to see.

Vanessa without proper documents the passport office will not do the amendments. Affidavit wont work without supporting documents. Since you already waited for two years this will be matter of two months to get everything corrected

Thanks a lot friends for all the information. But it’s really going to take such a long time. Already I’ve waited for two long years and nothing happened.
Is there no way where I can attest or make an affidavit since there is no amendments in her name only the middle name is missing in my passport.

Once you get her corrected passport you can apply for her name correction on your passport. I forgot to mention that. You will need her passport and Gazette copy for that.

I had the same issue for my mother. This is what I did.
1) I had her Goa birth registration doc with her correct name and her goa marriage certificate registration. I used those two documents to get her name corrected on her Aadhar. It took 3 days for the correction to reflect.
2) On the basis of her Aadhar and her Birth registration doc, I applied for her Gazette name change online. It took 7 days for me to get her Gazette copy. That was the most important document. With that, I could get her name corrected on almost everything.
3) I applied for her name to be published on two prominent publications and kept those newspapers. This took 2 days after the Gazette publication came online.
4) Then with the Aadhar and Gazette I got her name corrected on her bank statements.
5) Finally I applied for her passport with all the corrected proofs viz Gazette copy, Aadhar, newspaper publication and bank statements. I got her passport in 2 weeks. All in all it took me 3 weeks to get everything sorted.

On the sidelines while I had already applied for her passport, I applied for her correction on her pan card too which took the longest to get done. With just a little persuasion and patience you can get all your documents corrected provided you can prove to the authorities the genuineness of all your documents. Today everything is online, just take a day or two to do some reading and research, you will get all your queries answered.

PS. Church documents unfortunately hold no good anywhere above unless they are government notified verified and stamped.

You can try submitting the available documents to the passport office. If they accept well and good. Simultaneously apply for her adhar card with the correct name.It will take max 45 to 60 days for that.
