Antonio- There is no application form for registering a birth prior to 1961. The CRC after receiving documents will tick the documents received in its own form and will hand you one copy of the form. There is also no fee for rgistering birth pre-1961. So after you visit lisboa, please write your invaluable experience here for the sake of all your Goan bretheren.
Can anyone update which month process is going on for after 1961? or if anyone has lately received their Assento....?

I chose the one with less MB
RAPID SCANNER
There are many. If you have space on your phone. You can choose better scanners.
Crop the image accordingly. Avoid all empty spaces at the borders or if it is only half printed page, crop the blank portion out.
The image gets saved in the rapid scanner gallery.
Select and email it to yourself
Then open email and download the image in your selected folder.
Open consulate website
Attach the document from the folder you saved.

Read the instructions below the blank form. It says document number of assento de Nascimento to be filled for Citizen card.
Earlier last week it was not mentioned and I too had written my passport number and it got rejected for incorrect document number.
I resubmitted with assento de Nascimento Yesterday. But did not get any acknowledgement message. Was scared to resubmit. Because I read somewhere on this blog that application gets rejected for multiple submissions.
Quite nervous actually. Will wait for a week to hear from the consulate and will resubmit again.
