My name is Joao Ribeiro i have my process it has passed 2 years and 2 months, i sent an email on 26/09/2018 and Conservatoria replied on 27/09/2018 asking that if I declared if I want to mantain Portuguese or Indian Nationality and asked for my Indian Nationality Certificate. Did they asked these 2 documents to anyone in this forum?

Did anyone applied for New Passport at Goa consulate on or before 6 September and did anyone receive it ? Consulate said email after 3 weeks but until now no update

Yes you can add your surname approach the consulate with relevant name addition/change proofs they will do the needful.
Is it possible to add surname in my portuguese passport because i have only single name in my passport and every where they ask First name and last name and i have last name/apelido field empty.

Email them the CC/BI receipt that they give you on the day you applied for the CC/BI.
Mothers documents (Pre 61) were in order and sent in September 2017; one year now and still no registration, can someone advise the status or where the cases have reached.
