Dear Mr/Mrs,
I would like to find out as to what exact documents I will require for a tourist visa within your country. I will be going to Poland and staying over at my friend's house and visiting other countries such as Germany and Czech Republic. I am russian citizen who lives in UK. Please can you advice what type of visa I should apply for as I will be going only for maximum of two weeks and what documents will I need to submit to you? I will look forward to hearing from you.
