I know that the "Numer formularza" is the same as the number of the form which was created once I submitted the form, however, I don't know what is the "Otrzymany kod". Any idea what I need to put in its place?

Hello, my name is Jakub Stefan and I'm 18 years old. I am Polish ive lived in the UK for 6 years. Recently lost my passport and my birth certificate is there amt way i could apply for my passport back or anything. Please could you get back to me as soon as possible beacause i need my ID for my further education. Thank you very much.
I made visa appointment but I can not go to London on that day, I tried to cancel appointment but I did not find suitable information to fill in " Numer formularza" and "Otrzymany kod". I do not understand what they mean. If anyone can help me, please send email to me. Many thanks.
