I did not understand " Numer formularza" and "Otrzymany kod" mean. Could you help me that where can I find this information please.

hi everybody, i have made a mistake and i cannot fix it,i am Pakistani man and i want to go to Poland for Work for which I have a Polish Workpermit, i have erronously filled for a schengen visa but i needed a national visa, i wanted to apply for national visa on the Polish website but i received a message that i cannot use the same data to apply for 2 visas, i wanted to cancel the appointment of schengen visa but i cannot find the code, what is this strange code? can anybody help me please?
