I'm Egyptian and can't find the application form online or downloadable on the internet, where can I get the visa application form to be filled?

بيان بالمستندات و ما هو مطلوب لحصول مصريين علي فيزه سياحه للملكه تايلاند
1- باسبور صالح لمده سته اشهر علي الاقل من تاريخ الدخول لاراضي تايلاند
2- تذكره طائره مؤكد الحجز ذهاب و عوده
3- بيان بحسابك البنكي مبين به اخر بيان باخر ثلاثه شهر في تعاملاتك البنكيه علي ان يكون ختام الرصيد لا يقل عن عشره الالاف جنيه مصري
4- عدد واحد صوره شخصيه حديثه 4*6
5- تدفع قيمه استخراج فيزه السياحه و قدرها 220 جنيه
6- تسحب استماره و تملاها باللغه الانجليزيه
تقدم الاوراق للسفاره و تتسلم التاشيره بعده بثلاثه ايام عمل
تقدم الطلبات من الساعه 9.30 صباحا حتي الساعه 11 صباحا و عندما تاتي لتتسلم الفيزا تاتي الساعه الثالثه عصرا حتي الرابعه عصرا
هذه حدود معلوماتي لاني سافرت سياحه لهذه المملكه الجميله مرتان و كلما سنحت لي الفرصه ساذهب
ارجوا ان تكونوا قد استفدتم من هذه المعلومات
انا معى الفيزا الامريكية وانوى اسافر تايلاند كسياحة وعرفت من بعض اصدقائى انا حاملى الفيزا الامريكية يدخلون تايلاند بدون تاشيرة
هل هذا الكلام صحيح واذا كان لا فكم تكلفة استخراج الفيزا ؟وشكرا
I got my tourist visa for honeymoon from saudi arabia
but my wife didn't get it till now from egypt,
so i am asking about the duration needed for getting visa and the needed documents , i have already sent her a copy from my visa to submit it with her application.
Could she submit the document by her self or should be through a traveling company.
