You will to submit what they ask if you cannot find the Birth you can submit the marriage / Death record that will work too. Send the documents as soon as possible to save time. The file number doesn't change.

Well it's shouldn't be a problem, they understand that surname changes after marriage. Take one supporting document just in case needed.
