I just submitted yesterday, application for corrections on my son's birth certificate at andheri W, to correct my second name and to add my wife's second and surname. They have a requirement page on the notice board asking for any of the six docs proof namely PP, bank pass book, marriage certificate ( if married ), ration card, lic policy and election card. all should be prior to child's birth ( if it is child's birth certificate ) along with a request letter addressed to Medical officer(MO), I could only produce 2 of the above, but some one told me submit baptism certificate, school leaving certificate which i did, lets see how it works, have call my after 10 days. The clerk check and verifies with the originals and if found OK, initials the copy, informs us to submit in the dispatch, the dispatch clerk stamps the copy of the request letter and returns. I have been told if the MO finds OK then only the corrected certificate is issued or they may ask you to furnish some more documents. My advise you is to submit all documents which you have which have correct names. Affidavits are accepted.
Hope the above helps. Someone also told me that each ward has different requirement of documents.

Thanks, we are going through are Mother's side, yes, hope minor names should not create an issue and Mothers surname ( if different from husband ) will not match anyway with BC and Casemento, so i guess the CRC will understand about the change of surnames. Lets hope for the best, that's all we can do at the moment with all the bureaucracy especially in government establishment.

Your passport copy should only be of the front and last pages. They usually don't bother about anything else in between. The lack of visa on your passport is helpful. Good luck!
