Dear Sir
I in error applied for a national visa instead of a Schengen Visa. I am trying to cancel my appointment for a a national Visa so that I may apply for a Schengen Visa.
It asks me for two numbers 'Numer formularza' and 'Otrzymany kod', and I am not sure what these two numbers are, can you assist me or cancel it for me.

I have been trying for weeks now to contact the embassy to find out if we can send a courier to pick up the passport, but it doenst matter what time of the day you call, nobody answers the phone, and you leave a voice message and nobody calls you back...RIDICULOUS!!!!!!!!!!!!!!!!
