A note to Mr Bong
Please study my earlier comments on this website. I DISCOURAGE you to seek employment in Poland.
For consular matters contact Embassy in Kuala Lumpur. Here is the website: https://kualalumpur.msz.gov.pl/en/root Most likely you will not find any response from them.
I suggest other European countries- Germany, Austria, Holland and Nordic countries.

A warning
Job scams to Poland

Scams: Beware of job offers to Poland

The Embassy has received reports of Filipinos who are being recruited or have been recently recruited for promising jobs in Poland. Unfortunately, unscrupulous entities are taking advantage of Filipinos who are eager to work in Europe but who are not familiar with the proper recruitment procedure.

If you are being recruited for a job in Poland, and you are seriously considering an offer, please be guided by the following:

Direct hiring is discouraged by the POEA. As far as possible, hiring must go through a POEA-licensed Philippine Recruitment Agency (PRA), and the agency has to comply with the required documentation. Employment contracts must be verified by POLO Geneva before they are sent to the PRA to make sure they meet the minimum requirements on salary, hours of work, worker protection, and other labor standards. If you are offered a directly hired job by a company in Poland, please contact POLO Geneva immediately.

A note to Mr Lewandowski
Unfortunately you have a serious problem ahead of you. Embassy in Kuala Lumpur is totally useless so is newly opened embassy of Poland in Maynila. You can get relevant document from your local Civil Office in Poland. And you have to appear there in person. All documents have to be translated into English by a certified translator.
If you wish to provide location of your local Civil Office in Poland I can make preliminary enquiry.
.
Study earlier pages on this site. I responded to several people in relation to problems similar to yours.
