"Numer formularza" & "Otrzymany kod" is reqiured! but on my application form there is a one number, it may be the "Numer formularza" how can i find the "Otrzymany kod"?

Iam from Palestine Gaza i have acceptance letter from university in poland to study master . How i send my document to Embassy ,because of crossing it is very difficult to come to Ebassy hat is the way ? please help .
