Phone Poly at the embassy. She will provide you with an application form. You fill it in and send your application directly to the embassy with your passport. The Emassy is very efficient. In a day or so you will be called to collect. There is not charge whatsoever. Avoid all the middle players trying to make a bid buck out of you.
