Dengan hormat,
Dengan ini saya ingin menyampaikan permohonan bantuan dana sosial kesehatan untuk ibunda tercinta, pada tahun 2012 bulan desember beliau menjalani operasi amputasi pada ibu jari kaki dan hingga saat ini beliau belum bisa berjalan akibat tulang tumit bengkak, saya sangat berharap ada Bapak / Ibu Dermawan yang bersedia dengan ikhlas membantu saya, karena kami tidak memiliki biaya untuk membawa ibu ke dokter ahli tulang atau bedah,saya sangat berharap atas bantuannya.
Demikian permohonan dari saya, atas perhatian dan kesempatannya saya sampaikan terima kasih.
