I live in the Bahamas and my passport expired last year. I have downloaded the forms but need the DHA-9 form (fingerprint) Can I get this form from you? If so what is your address so that I can send the envelope for you to send the form to me.

Good day, I am a trained teacher holding a degree in education. I am very much interested in living and working in South Africa and therefore I'm seeking your advise as to the way forward.

I'm a South African currently working the the Caribbean on a long term contract and my passport will be expiring in the next 12 months. I would like to apply for a new passport and dont believe I will be able to go back to South Africa before it expires. I have downloaded the application forms from the website but dont have the DHA-9 form (fingerprints).
1) Can I apply for my new passport at the embassy in Kingston
2) Can you do my fingerprints at the embassy as I dont have the DHA-9 form nor is it downloadable from the internet.
2) I have an original birth certificate (Unabridged copy)that was apostatized in 2008. May I use this for my application or may I present the original at the embassy in Kingston as proof?
In terms of birth certificates and marriage certificates, can we have the originals certified and then use the certified copies for the application so we are still left with the original copies?
Keen to hear from you soonest. I want to come to Kingston to make the application.

I am a Jamaican citizen and i'm in the island for christmas holidays. While I am here I would like to start taking the necessary vaccinations for the trip.
