Duly filled in application Form
Original expired passport along with copies of first four pages
Original CNIC or NICOP along with a copy OR Notarized copy of CNIC or NICOP
For children below the age of 18 years and not having NICOP, please attach original Children Registration Certificate (CRC) OR a notarized copy of CRC
Original or notarized copy of P.R. Card or Citizenship Card or Work Permit or Student Permit or birth certificate for children born in Italy.
Three identical most recent colour photographs duly stamped by a photographer in Italy, taken within past three months
A certified cheque or money order in the name of “Consulate General of Pakistan” for the amount mentioned in the fee table
Applicants sending their applications by mail please do enclose a prepaid self addressed courier or registered mail envelope

Police Report of the lost passport
Affidavit of lost passport
