Please advise what is the process? where do we get the relevant forms?

please advise where in this earth one can download forms for renewal of passports. I have tried on the only site I found it I am blocked from that site by my computer stating its a risky site.

Please advice what is the process to obtain a new passport ( mine is about to expire) what forms need to be completed, where to get the forms and where to send the completed forms, and how much it costs now and can I pay with Debit/Credit card?

The service at the Capago Sandton offices was brilliant! Probably because I was helped by very competent South Africans.
I don't really have words for what I experienced at the Italian Consulate today. I think it would be a waste of my time and energy to try and put my experience into words. I am proudly African and I am very happy that my French ancestors decided to leave France and make Africa their new home.
Some of my late grandfather's best friends were Italian and he was fluent in their language. I remember them well and the individuals I dealt with today was nothing like them. The people I saw today were rude, cold, condescending, chauvenistic, disrespectful of South African traffic rules and regulations, lacking the ability to speak basic English fluently and much more. According to them they lived behind high walls in a "little Italy" in the heart of Houghton. They were also above the law and had to answer to nobody.
After my visit I truly have no desire to visit Italy. One would think that a country that is struggling economically would welcome tourists with open arms. Instead they look like they think they are doing you a favor by issuing you with a visa to visit their country.
It was, without a doubt, one of the worst experiences of my life.
Iam deeply disappointed'in the manner in which my visa application application was handled, I, went to the Capago Sandton offices on the 8th of July to apply for my Italian visa for my birthday trip. I went for interview prepared will with all the check listed documents.
To my dismay i was informed on the 11 July 2016 that my visa had been refused due to me not having adequate valid insurance, at first I, was perplexed because my medical insurance covered more than what was required, upon closer inspection of the medical insurance I, discovered that the insurance company had incorrectly stated the wrong month of my travel, JULY instead of AUGUST was stated.
I,was informed that I, could reapply only in 6months or I had an option of Appealing the decision of which the Appeal process needs to be done before the TAR in Rome by Italian lawyers. Out of desperation I, made inquiries to a Roman firm which quoted me a fee of 1500 euros and said it will take anything from a month to 4 month for the case to be heard in a Roman court and that its a WINNABLE case due to the fact that the Consulate is/was obligated to inform me of the wrong date on the insurance so I, could submit a correct one before issuing a negative act/decision.
I was never requested to submit a correct insurance and to make matter worse, at my interview the ERROR was not even noticed by the Capago Agent who is supposed to check all documents if they are correct.
My insurance has amended the incorrect certificate and reissued me with the new CORRECT one and I, have even gone an extra mile of getting a top-up insurance.
I, dont have the money to pay for the appeal and even if I had, i don't have enough time before my travel for the process of the appeal to take place.
All flights and accommodation has been issued and confirmed and are non refundable, I stand to loose R25 000 because my insurance does not cover claim of visa refusal
More than anything am disheartened for the fact that this was a birthday celebration trip for my 40th birthday and to add salt to the wounds am going to loose 25K, what a sombre state of affair, what a way to celebrate!!!:(
A refusal for something that could have been/has ratified. I think you're to harsh in the implementation of your policies they do not have any humanitarianism and the fact that you disregard your own mandate...to inform
