Please kindly approve I got my visa Italian State for the purpose of complementing treatment build ANAS where already been processed in one of the hospitals in the Italian city of PAVIA (Hospital SAN MATCO) in 2006. Where he was suffering from a tumor and the Valley was a surgical intervention at the hospital mentioned above and under the supervision of Dr. (MARTUCCIELLO) and after the end of the recovery period back to Iraq for the purpose supplement treatment as to build ANAS currently suffering from some problems in the place of surgery and some complications.
Having been contact your doctor been invited from physician need attending to Italy and that after studying the X-ray analysis and modern which I have made for my son and quickly confirmed attendance for the purpose of supplement treatment, according to its guidance.
So I ask you to consider my request for ambulance child. Note that the call from the doctor and the hospital are on your website your embassy for the purpose of you are aware of the case build ANAS. In addition to a copy of the passport and the call and thank you.

ارجو التفضل بالموافقة على حصولي على تأشيرة دخول الدولة الايطالية لغرض تكملة علاج أبني ANAS حيث سبق ان تمت معالجته في أحدى مستشفيات مدينة PAVIA الايطالية ( مستشفى SAN MATCO ) في عام 2006 . حيث كان يعاني من ورم ولادي وتم اجراء تدخل جراحي في المستشفى المذكورة أعلاه وبأشراف الدكتور ( MARTUCCIELLO ) وبعد ان انتهاء فترة النقاهة عدنا الى العراق ولغرض تكملة العلاج إذ أن أبني ANAS يعاني حالياً من بعض المشاكل في مكان العملية الجراحية ومن بعض المضاعفات .
وبعد ان تم الاتصال بالطبيب الخاص تم توجيه دعوة من الطبيب المختص بضرورة حضوري الى أيطاليا وذلك بعد أطلاعه على التحاليل والاشعة الحديثة التي اجريتها لابني والتي اكد فيها بسرعة الحضور لغرض تكملة العلاج وحسب ارشاداته.
لذا أرجو من سيادتكم النظر في طلبي من اجل أسعاف الطفل . علماً بأن الدعوة من الطبيب والمستشفى موجودة على موقعكم الالكتروني الخاص بالسفارة لغرض اطلاعكم على حالة أبني ANAS . أضافة الى نسخة من جوازات السفر والدعوة وشكراً .
