HOW CAN I RENEW MY ISRAELI PASSPORT?
I am an Israeli citizen living in NY currently would like to have an Israeli Citizen form which I am unable to find it online, I appreciate it if you send me the form or the link:

A dog puppy of friends found his way to my bag, took out my passport out of it and started to chew it.
The page with the picture is not damaged at all, but the page with the signature is a bit torn , and other pages were torn off, and also the edge of my visa page the part with the numbers is a bit torn.
Can I use my passport or should I ask for a new one , and if so what about my visa , can i still get it in my new passport ?
Would love to get some answers but since i could not reach yet any human being on the phone , I'm trying to get help here. Thanks for any help in advance :)

No one answers the phones, nor they return calls when you leave a voicmail and number to call back

Prince Andrews father princePhilip has persecuted us ever since we where small children by calling everyone here below "sick".
Prince Charles henpeck people and says I shall get hurt through my son. Prince Charles is obvious à case for mental care.
Queen Elisabeth is very "disturbed" and hurts my innocent son when she can't hurt me when I says her son prince Charles is very evil.
